New Culinary Students Forms

  Student Petition

All Culinary and Pastry Arts students must submit this form during their advising sessions each semester. If a student fails to submit this form, their desired courses will not be unlocked for registration.

Student Petition – Spring 2018

 

  Food Handler Certificate

First-time Culinary and Pastry Arts students must submit a current Food Handlers Certificate from a State Approved course or a course that is accredited by the American National Standards Institute (ANSI). This will give you a base-line knowledge of food safety as you enter the lab.

Current & Former Culinary Student Forms
