New Culinary Students Forms

  Student Petition

All Culinary and Pastry Arts students must submit this form during their advising sessions each semester. If a student fails to submit this form, their desired courses will not be unlocked for registration.

Student Petition – Summer/Fall 2017

 

  Food Handler Certificate

First-time Culinary and Pastry Arts students must submit a current Food Handlers Certificate from a State Approved course or a course that is accredited by the American National Standards Institute (ANSI). This will give you a base-line knowledge of food safety as you enter the lab.

Student Petition – Summer/Fall 2017

 

  Program Evaluation

Current/Former Culinary and Pastry Arts students must present their Program Evaluation along with their completed Student Petition at the time of advising.

 

Follow these steps to print your Program Evaluation:

  1. Login to Online Service
  2. Under the Academic Planning section, click Graduation Application
  3. Click Run Program Evaluation (Found in the blue box.)
  4. Select your degree shown in blue box
  5. Click Submit  (DO NOT select a degree from drop-down menu.)
  6. Your report will take a moment to generate
  7. When your report window pops up, Right-click to Print report
  8. Click Close Window at the bottom of the page
  9. You will return to the Graduation Application page, DO NOT click Continue to Application
  10. Click Log Out at the top of the page

 

–  Food Handler Certificate

Current/Former Culinary Arts Students should have a valid certificate on file. However, if the certificate has expired, students will need to retake a food handler course for an updated certificate. See New Culinary Student Forms section above for online course options.
