  8. How many hours do you plan on working while enrolled at ACC?
  9. Are you a U.S. Veteran?
  10. Number of persons in your household (ONLY if there is a demonstrated need/use for this at ACC. Otherwise the committee recommends removing it.)
  11. Do you have children under 18? (ONLY if there is a demonstrated need/use for this at ACC. Otherwise the committee recommends removing it.)
  12. Reverse transfer question/release of transcript for possible future four-year university.
  13. Meningitis information (Required by State of Texas Statutes)
