If you already submitted the immunization form through CastleBranch please notify the Health Sciences Office when submitting your documents.\u00a0<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Download the <\/span><a href=\"https:\/\/drive.google.com\/open?id=17wIxa25hZfBGCeCr2I1kEMd1oRWmdIb4\"><b>Immunization and Tests Required Form <\/b><\/a><span style=\"font-weight: 400\">(PDF)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>The form can be signed by any licensed healthcare provider and must include their credentials.<\/strong> It does not have to be the person that gave the immunizations.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">If you have already created an immunization account for ACC Health Sciences or ACC Continuing Education with CastleBranch, you do not need to pay for the account again.
