If you are accepted into the program, you will be required to upload your <strong>pre-approved ACC Immunization Form<\/strong> into the <a href=\"https:\/\/sites.austincc.edu\/health\/complio\/\"><span style=\"text-decoration: underline\">Complio<\/span><\/a> system as part of your next steps.<\/p>\n<p><span style=\"text-decoration: underline\">Instructions<\/span> for completing this process will be provided upon <span style=\"text-decoration: underline\">acceptance<\/span>. If you are not selected for admission, no further action is required and you will not need to upload your immunization form to <a href=\"https:\/\/sites.austincc.edu\/health\/complio\/\"><span style=\"text-decoration: underline\">Complio<\/span><\/a>.<\/p>[\/vc_message][vc_message]<h3>Mandatory Submission Requirements:<\/h3>\n<p>When submitting your immunizations to Complio, you must include the following information:<\/p>\n<p>\u2022 Full Name<br \/>\u2022 ACCeID<br \/>\u2022 Assigned Program<br \/>\u2022 Date of Birth<\/p>\n<h4>Important: Complio will reject your submission if any of this information is missing. Please ensure your account profile is complete before uploading documents.<\/h4>[\/vc_message][vc_column_text css=&#8221;.vc_custom_1777064105262{margin-bottom: 20px !important;}&#8221;]<p><span style=\"font-weight: 400\">Health Sciences students must comply with both Texas law and clinical facility requirements related to immunization and testing. <\/span><span style=\"color: #ff0000\"><b>Students should not delay completing their immunizations; some take up to 4-6 months to complete.<\/b><\/span><\/p>\n<p><b>Students must submit the ACC immunization form directly to the Health Sciences Admissions Office at <a href=\"mailto:healthsciences@austincc.edu\">healthsciences@austincc.edu<\/a>. If you already submitted the immunization form through CastleBranch please notify the Health Sciences Office when submitting your documents.\u00a0<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Download the <\/span><a href=\"https:\/\/drive.google.com\/open?id=17wIxa25hZfBGCeCr2I1kEMd1oRWmdIb4\"><b>Immunization and Tests Required Form <\/b><\/a><span style=\"font-weight: 400\">(PDF)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\"><strong>The form can be signed by any licensed healthcare provider and must include their credentials.<\/strong> It does not have to be the person that gave the immunizations.<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">If you have already created an immunization account for ACC Health Sciences or ACC Continuing Education with CastleBranch, you do not need to pay for the account again.
