Retention, Archiving, and Disposition of Protected Health Information (PHI)

PHI will be retained according to ACC’s Record Retention Schedule which adheres to state and federal regulations pertaining to HIPAA.

Retention Requirements

  • PHI documents will be retained for a minimum of 6 (six) years.
  • PHI disclosure documentation will be retained for a minimum of 6 (six) years.
  • Complaints filed regarding privacy rights violations will be retained for a minimum of 6 (six) years.

Storing Inactive Records

Storing of inactive College records will be maintained as outlined in College’s Procedures 3.03.014, Records Center.

Disposition of Records

Disposition of College PHI records will be carried out in accordance with applicable state and federal laws, the College Records Retention Schedule, and College’s Procedures 3.03.014, Records Management Program.
