I already paid for the immunization account, do I need to pay for another one?
No, the immunization account applies to ALL of our programs. You only need to pay for it once.
Can I upload my personal records?
No, all immunizations need to be on the ACC Immunization Form. CastleBranch will not accept personal records.
Who can sign the ACC immunization form?
Any licensed healthcare provider can sign and date the form. It does not have to be the person that gave the immunizations. Be sure the date the form is signed is on or after the last immunization date.
Why can I not sign the form myself?
The healthcare provider is verifying that all of the information on the immunization form is correct.
Do I upload the same ACC immunization form to each requirement?
Yes, you will attach the same form to each of the 4 immunization requirements.
I have submitted the immunizations to My Documents. Do I need to do anything else?
Yes, anything in My Documents can only be seen by you. You will need to attach the immunization form to the 4 requirements in your To Do List.
What do I need to do for item 1 in the immunization account?
You will need to check the box to show that you understand only our immunization form can be submitted.
What if I can’t find my immunization records?
You can contact the Health Sciences Compliance Coordinator at [email protected] who can look for records in the statewide registry. Other options are to redo the immunizations again or have bloodwork done to check for immunity.
How do I ensure my immunization form has been submitted correctly?
Please check your CastleBranch account for the status of your immunizations. A green check means approved. If rejected, please read the reason CastleBranch provided or contact us.

TEAS Testing Requirements for Health Sciences Applicants
A TEAS Exam score is required to apply to the following programs: Dental Hygiene, Occupational Therapy Assistant, Radiology, and Vocational Nursing.
If you are not applying to one of the programs above, then you are not required to take the TEAS Exam at this time. Please refer to the Application Process page for your program to see all application requirements.

All students who have not been accepted into an ACC Health Sciences program should declare General Studies in Pre-Health Sciences (Associate of Science) for their major. Once accepted into a program, the student's major will be updated.
