Students must be successful in the lecture/laboratory course (PLAB 1323/1023) before being allowed to attend clinical.

The Phlebotomy Technician class is only offered at the Eastview Campus.

The criminal background check must be submitted at the same time as the application.

The Phlebotomy Technician application can be submitted with 2 of the 3 Hepatitis B shots completed and the 3rd completed before the semester begins.

All applicants are required to submit documentation of a negative drug screen with their application.
