Important:

This approval is for application purposes only. If you are accepted into the program, you will be required to upload your pre-approved ACC Immunization Form into the Complio system as part of your next steps.

Instructions for completing this process will be provided upon acceptance. If you are not selected for admission, no further action is required and you will not need to upload your immunization form to Complio.

Mandatory Submission Requirements:

When submitting your immunizations to Complio, you must include the following information:

• Full Name
• ACCeID
• Assigned Program
• Date of Birth

Important: Complio will reject your submission if any of this information is missing. Please ensure your account profile is complete before uploading documents.

Health Sciences students must comply with both Texas law and clinical facility requirements related to immunization and testing. Students should not delay completing their immunizations; some take up to 4-6 months to complete.

Students must submit the ACC immunization form directly to the Health Sciences Admissions Office at [email protected]. If you already submitted the immunization form through CastleBranch please notify the Health Sciences Office when submitting your documents. 

  • Download the Immunization and Tests Required Form (PDF)
  • The form can be signed by any licensed healthcare provider and must include their credentials. It does not have to be the person that gave the immunizations.
  • If you have already created an immunization account for ACC Health Sciences or ACC Continuing Education with CastleBranch, you do not need to pay for the account again. The immunization account applies to all of our programs.
  • Students should have immunizations submitted at least 2-3 weeks prior to application deadline to ensure they are complete at the time of application.
