TB Test Information
Incoming New students/faculty:
- TB skin tests no longer accepted.
- QFT or TSPOT blood test within 90 days of the start of the clinical semester which will be repeated annually at the direction of your department.
- Students/Faculty will submit a copy of their lab report that includes negative results from the QFT or TSPOT blood test. The report must also include name, date of birth and result date of the blood draw.
- TB questionnaire will also be submitted before the start of EVERY semester.
Prior/Current positive blood test:
- Submit positive blood test results ALONG WITH a clear chest x-ray report (lab report) administered within the last 3 months along with the TB Questionnaire as directed by your department.
- Fill out the TB questionnaire portion of the form EVERY semester.
CastleBranch:
- Package Code YE68dm
- Students/Faculty will provide an explanation of all “Yes” answers on the TB Questionnaire form. If there is not enough room, please attach an additional sheet.
- CastleBranch will reject any questionnaires with a “Yes” answer.
- Lisa Enloe will review all questionnaires with a “Yes” answer.
- If questionable results, will refer the student to meet with their HCP.
- Students/Faculty will need to provide documentation of TB clearance.
- Lisa Enloe will approve in CastleBranch.
